Terms of Use

I, the client, legally agree to all of the following terms of the online scheduling system, hereafter referred to as "system", and with regards to my business and health care relationship with InnerLight Massage and Colon Hydrotherapy/Massage by Chad hereafter referred to as "InnerLight". I realize that I have a choice to not agree to these terms and to choose another health care organization for my health care.

Practitioners at InnerLight are not medical or naturopathic doctors. They do not offer medical diagnosis, cure, advice or treatment for any medical disease ailment, injury, infirmity, deformity, pain, or other physical or mental condition. Practitioners do not prescribe or recommend discontinuance of any prescription drugs. I understand I use this system at my own risk and it is my responsibility to only use the system if I understand what I am doing. This system is being offered as a convenience and I do not need to use it to book appointments. Reminder emails, as well as reminder calls, are a courtesy and whether or not I receive them I am still liable for appointments that I make. I understand that InnerLight does not staff a help desk to support this system outside of Monday through Saturday 10 am to 6 pm. I understand that if I have problems with the system it may take up to 24 hours, or longer (especially during a weekend), to get a response.

Unlike doctors, InnerLight does not/can not schedule several clients in the same time slot. I realize the time I book with an InnerLight practitioner is reserved specifically for me the client. If I, the client cancel on short notice or forget my appointment, I realize I have waisted 1 1/2 hours of the practitioners time. I also agree that it is very difficult to fill that time slot with less than 24 hours notice. I, the client will respect the practitioners time so that the practitioner may respect mine.

I understand that since I am new to the booking system and due to the possibility of error, if I am at all unsure of how to book an appointment that I should call 801-366-9544 and ask for assistance in the booking process.

If I book an appointment, even accidentally, then I am financially responsible for that appointment.

I understand that I will pay for my sessions at the time services are rendered.

Rates may change without notice.

I agree to cancel and/or reschedule my sessions at least 24 hours in advance of my scheduled appointments by calling or leaving a voice mail message at 801-366-9544. If I fail to do so or don't show for my session I will pay the full session fee in person, by mail or via an email invoice that will be sent to me. If I fail to pay within 10 days of receiving the invoice then InnerLight has the right to send me to collections. Click here for the full details of the cancellation policy. The online scheduling system will not allow clients to make any changes to appointments during the 24 hours prior to a specific appointment.

Cash, checks, and credit cards (Master Card, Visa, Amex and Discover) are acceptable methods of payment.

There will be a $30.00 fee charged if my check is bounced due to insufficient funds.

If a client fails to pay for missed appointments or any other fees due and owing then said client will be restricted from booking future appointments until fees are paid in full.

I understand that the information divulged during an appointment is confidential and that the practitioner will protect my confidential information.

Privacy & Protection: All information gathered in the system is maintained through an encrypted and secure server. The clients privacy of personal and health information is safeguarded according to HIPAA government regulations. No other clients using the system will be able to view my bookings or personal information. Only the internal authorized staff of InnerLight have access to the booking system.

I authorize InnerLight to contact me via the contact information I provide for any reason related to my relationship with InnerLight.

I authorize InnerLight to collaboratively support my health needs with my other health care providers as I disclose verbally or in written form or to those health care providers that have referred me.

I know that I am responsible for my own health and actions and only I can heal myself. Colon hydrotherapy, massage, nutrition counseling, health care products and my practitioner are only tools I employ to help me. I initiate services here with this in mind.

I understand that my practitioner is not a doctor or licensed psychotherapist and any therapies, conversations or sessions used to support my wellness do not infer or intend to diagnose, treat or cure physical, mental, emotional or spiritual disorders.

Before receiving colon hydrotherapy/massage therapy, I agree to always review the contraindications, and provide a copy of a prescription from my doctor to my practitioner regarding any contraindications that apply to me.

I understand that the "Terms of Use" of using the Online Scheduling System may be updated without notice to further protect all parties involved. I understand that I am bound to the terms posted publicly at the time appointments were made. I agree to review the terms before each new appointment to ensure my understanding and compliance. If major changes are made I agree to allow InnerLight to email me with these changes if they so choose.
